When a community puts effort into something, this tool helps see whether it's making a difference. The answers go back to the community, not up to an office.

How this works

Name what you're assessing — a program, a project, a plan. For each area, consider:

What were the goals? (What was this supposed to accomplish?)
What's actually happening? (What's falling short?)
What should change? (What would make it better?)

Traffic lights make the answer visible. Green = working. Yellow = needs attention. Red = action needed.
